Please note that you are permitted to apply to only one academic program per application cycle. Incomplete applications will not be reviewed.

Physical Therapy Applicants: You should not submit this form to apply to the Doctor of Physical Therapy Program. You will receive an email upon submitting your PTCAS application. Once submitted, your data will be transferred to UCSF, and an automated message will be sent to the email address you provided in PTCAS. An application will automatically be started for you. It may take up to 72 hours for us to receive your application information.

The application cycle for 2025–2026 is now closed. The 2026–2027 application cycle is expected to open in early September 2025. Please check back at that time for updates.
